New Patients

In order to facilitate case transfers, please complete our referral form to consolidate the pertinent history, laboratory and prior treatment data.  You can send this form electronically, fax it to (425) 823-4008, or have the client bring the form directly to the hospital.  This helps us help your patient in a more informed and efficient manner.  To further increase communication and continuity of care, please call ahead and speak with one of our emergency doctors or specialty referral coordinators regarding care transfers.
